LF Nightfall only PvE build

Well Ive been out for over a year and decided to get back into playing. As stated above, I only have Nightfall so that limits me a bit. But Im about done with the campaign and Ive got a feeling my build could be alot better than it is. Im not really fluent with skills, effects etc. in the game so I thought I would turn to you guys.
Here's what I have now;
Reaper's Sweep - Chilling Victory - Rending Sweep - Reap Impurities - Vital Boon - Natural Healing - Healing Breeze - Resurrection Signet
Skills;
Healing 4
Scythe Mastery - 16
Wind Prayer - 7
Earth - 11
Mysticism - 2
Also, I wouldnt mind dropping the monk profession if there are any other skills that would come in handy.

Mystic Sweep, Eremite's Attack and Heart of Fury are all useful skills.

In general, you should have no more than 3 attributes at one time. 4 or more just leaves you watered down and not getting much out of your skills - Healing Breeze is bad at 12 Healing, let alone 4. Ditch it and pump up Mysticism. More energy management means more skill spamming means more damage.

If you're able to change secondary professions yet I would switch to either /ele or /war. People use ele to use conjures and others use war for attacks like wild blow and protector's strike.
For the build I would suggest something more like,
[reaper's sweep][eremite's attack][chilling victory][mystic sweep][heart of fury][attacker's insight][conjure frost](or a different conjure][Sunspear ressurection signet].
Melee professions should have an IAS which is why dervish's bring heart of fury. Eremite's attack and mystic sweep are both good because of the activation times (3/4 seconds in pve) meaning quicker attacks.

DROP HEALING BREEZE.
First of all, you have 11 Earth Prayers.
That's definately good for Mystic Regeneration.
Much better for Dervishes.
Also, swap out Natural Healing for Dwayna's Touch.
I'd also swap out Rending Sweep for Victorious Sweep.
And, swap out Res Signet for Mystic Vigor.
That's if you feel comfortable with it.

Alright, thanks guys for the help so far. Im gonna switch out for Victorious Sweep once I get it and maybe go with Wild Blow.

Good depending on the area.
Melandru is good for condition heavy areas
Dwayna for Hex-heeavy and self healing
Lyssa for areas with lots of enemies with long activation times on skills (Elementalists, Necros, Rangers with Troll Unguent)
Grenth is good for more end-game areas, as the high life stealing coupled with something like Nightmare Weapon makes for great damage and self-healing

There is also AoB, which is considered by this community to be the worst of the avatars due to the fact that it's effects can be simulated without elite skills (though the only way I can think of to get past the armor cap of +25 would be with something like Conviction + I Am Unstoppable; the latter being an EotN PvE-only skill). I think it gets a bit of a bad rap, though, because no one mentions the fact that to do so you would need to use up 3 or 4 skills (depending on your build), whereas AoB requires only itself and Eternal Aura to maintain it. Still, I'd recommend the other avatars first, if only for the fact that a warrior can still outtank an AoB dervish.

But Im like two missions away from beating the campaign and running into BIG trouble with holding up against enemies. Is it still my build or do I need a new scythe?

How high is your lightbringer rank? The higher it is, the more damage you deal to (and less damage you take from) demons. Rank 4 should be high enough, but 2 probably isn't. And make sure you've got it up.
Don't forget about your heroes' builds. At this late stage in the game, they need to be at least decent as well.

Those Dryders can spike pretty much everything if you're not careful,... Prot'ting is key. Make sure you've got a Hero Hybrid Monk or 2 on your team, and you should be fine.
