Don't know any bugs, but if you do a search, there are LOTS of posts about cartography in all the games that would help you out. A few tricks exist in Tyria to reach places not normally accessible. For instance, in The Frostgate mission you can go along the same path Rurik does to open up a decent amount of map. Do some searching and you will find a guide somewhere on Guru as well.
Yes & no, if you get textmod just make sure it's the legitimate one (don't have the download site, but maybe someone can post). There are 'alternate' versions available, which is to say they've been altered and can have keyloggers.

I use Textmod all the time. Don't know how I could live without it. I've never had any trouble with it, but then again, I've had it for a long time :P
Cartographer is a great mod for Textmod, but if you're iffy about using it, then check around some sites for maps. I'm sure there is some sites out there that have the maps of all areas so you can compare them and check what you are missing.
edit: btw, don't forget that some cutscenes reveal some of the map as well. So make sure you're not skipping any cutscenes. :P

Unless we can look at your map, there's not really much we can help with. There are inaccessible areas that you can get to in Frost gate, and also some in the Northern wall mission that help quite a bit. And then there are also hidden areas like the area behind the tree and the Xunali chests in the Aurora Glade outpost.
My best recommendation would be to get Texmod from the Wiki and run the cartography mod. While others might have had bad experiences with it, I have been using texmod for over a year now for various parties, mods, and especially for Cartography. I've never had a problem.
