arch psyker
both factions and nightfall added in additional hairstyles for all core classes.
you can check all available styles by going to the Great Temple of Balthazar and talking with Anatomical Engineer Llye.
Hope that helps out.

These should help you out.
And yes, the makeover packs cost money. $10 USD for 5 changes. They're great for people who have invested a lot of time + effort into their chars and want a new look, or for people who want an appearance that is, for example, a tyrian face, elonian hair and canthan hair colour.
If you know you're going to be creating a Factions necro with factions hair with factions hair colour / skin colour, then yeah, you'd be better off just getting Factions. Plus you get new content to play then too!

Do you even need to own the campaign that you're trying to get hair or skin color from? I'm looking on a spare account with only prophecies and it has all of the options, but I'm not sure if it will give some error message if I would click confirm.

@Neo -- according to guildwars.com you are only shown changes that are available to you.

Q: How and where can I use my Makeover Credits?
A: Go in-game and travel to the Great Temple of Balthazar, located on the Battle Isles. Speak to Anatomical Engineer Llye [Stylist]. This NPC will provide details of the hair styles, facial features, skin colors, and height options available to you. Changes to the character's appearance will occur when you apply the selected alterations, and a Makeover Credit will then be deducted from the account.
