there probably is. Probably a side effect of one of those perfect damage mods with no req.

there is an old presearing axe with -20hp. It's nearly IMPOSSIBLE to find, as a lot of people on this forum are unsuccessfully looking for one.

no other -hp weapons exist

The cities of ascalon quest there is a grim cesta with -50hp. Solo/55 monks use it. I also found a staff the other day in my inventory i must have gotten from a quest it had health +25, health -20 which was bizarre i thought.

Ugh, if anyone had such a weapon, we'd have 35Hp Monks. It would sell for more than anything else in the game. Yowsa!

I think the OP is looking for a 'weapon' not an offhand item. These are much harder to find.
