I've tracked deaths/Charr levels in DL sessions for LDoA; had also used a checklist for Elite Skills that needed capping while going for GWAMM.
I also have a "To Do" list for HoM, another GWAMM, etc.

Did the same with the Elite Skills, printed them out and crossed them out as I captured them.

I dunno if this counts, but back before I got Proph and NF, I planned out all my future characters... looked up names, drafted personalities, wrote them down in the back of my Factions manual in the "notes" section.
I also printed out the GuildWiki elite skills checklist and keep that updated so I know what to cap while I'm in an area, and what I still need to get (so close to Tyrian Elite Skill Hunter now...).
Oh, and I keep treasure checklists for all my characters in NF on my GuildWiki userpage, so I know when they'll be ready for me to open again. I check them all every couple of months or so.
When I am bored in class, GW things tend to happen. I've been known to:
1. Make checklists of what I still need to do
2. Doodle my characters
3. Doodle my characters' children
4. Sketch thumbnails of GW commishes I need to work on
5. Make preparation sheets (this was before I started to play - I printed out images of the armors I wanted, and wrote notes down of my plans for the charas' builds, appearances, backstories, etc)
6. Make character sheets of my characters (actually that only happened once... it was a "what if Gui were a henchie" sort of thing; I actually wrote about 10 or so idle quotes for her and all)
7. Make UW readiness sheets (I still have those somewhere... these were from when I'd been playing GW a couple months, hadn't beaten Factions yet - I remember the builds I wrote down and they're pretty humorous)
8. Make GW2 prep sheets (yep, planning out GW2 charas already)
9. Sketch my GW2 characters
10. Design builds (for myself and guildies)

I was considering writing an actual (in-character) journal on my Para or something, but I haven't played her in forever so yeah. Maybe in GW2? Since we won't have all the info up and refined on wiki for a while after release... it would be nice to have notes, and if they're in-chara I can put it up on Guru as fanfic.

I have a number of notebooks that I've scribbled loads of stuff in. Things like material costs, skill duration calculations, noting people's real name with their character name (yeah I like to know who I socialise with, bit of a weirdo), preparing for guild contests, keeping scores in guild contests etc.
Other than paper I have a spreadsheet I downloaded for skill caps, and about 4000 screenshots of bits of guild wars (most are weapon stats for trade though). I also have a 1Tb hard drive half filled with in game footage I've been recording since early 2006 which I recently made into a video semi chronicling, semi advertising the Guild I founded way back then. (Link: http://www.youtube.com/watch?v=wVUL6WC5ioA ) The raw footage is cool to look at for a trip down memory lane ^^

I have sheets of paper all around my room of XTH popular picks of people in the know. Then I'd cross reference them and figure out what the general consesus on rankings were.
I also used to have sheets of Builds before the whole bar pinging thing was avaliable. Other than that, the only notes I still have is my orginal GW Proph manual, I love reading that story about defending the mages and such.

I've got a list of elite skills, that I check off whenever I cap one. I want to have all skills of a profession of every character of that same profession, and that list helps me. (the non-elite skills I just check with a priest of balthazar and a normal tome of each prof.)
Then I've got a list of all the pets I want to charm and level to 20 for my menagerie.
I've got a big list for all missions I've done and still need to do to get protector on all my characters, including EotN.

Other than keeping track of money titles(sweet, drunk, party) in a .txt file, all of my notes are upstairs. The few hundred screen shots I have consist of some of my earlier accomplishments(like beating Glint the first time) or the hundred or so images I have of Henge of Denravi where there was a small spot I couldn't uncover for carto.
Before ANet implemented templates, I used my screenshot folder to record builds and name them.
I used to take notes before I found wiki on NPCs for rare crafting materials, collector armor, etc.
I made a spreadsheet of the Vabbi quests that awarded trade contracts along with the characters able to do them and which were completed.

Back before skillcode was implemented, I'd write down skills and attribute points so I wouldn't forget fun builds.
I also did a checklist for skill hunter and the locations of the easiest to acess bosses, since I'm lazy and didn't want to constantly check wiki.

I take a lot of stuff from guildies but I have spreadsheets for 17 toons and all their heroes with weapons/armor/rune/insigs/b-days/various titles track levels... I have full inventories of pack mules carrying greens, collector items, upgrades etc. Another with the family trees in my alliance. I keep them in a binder.
Now that I have typed this, it seems my spread sheets are a lot like work. Someone help me...
