Do you get one chance at this title or can it still be achived after dying?

Depends, there are sometimes where you can die and it doesn't count, for example during the tutorial.
But in general if you die, it's game over for the title.

If your death count goes from 0 to 1, that's it. There aren't any second chances, except for re-rolling the character.

Okay so basicly it works till you are lvl 20.

Use heroes/henchmen and flag them forward so they take the initial aggro. Try using spells on your assassin instead of dagger attacks, you'll be much safer (assacaster). Getting to level 20 is just the start of the survivor title. Level 20 is rank1 with about 140,000 xp. The final rank is rank 3 with around 1,337,000 xp, about 10 times level 20. You need to achieve this with 0 deaths on your /deaths counter.

If you only want to rush to Gunnars Hold for grinding XP in the brawling dungeon, then consider a different approach: find a rusher that brings you through zen dajiun, then to eotn, and then to gunnars hold. Failsafe, but completely without challenge, fun and honor.

The main way for an Assassin to stay alive long enough to reach level 1 Survivor seems to be to invest in Shadow Arts - Shadow Refuge, Death's Charge and/or Heart of Shadow for quick recovery from Afflicted explosions in the Lagoon and Zen Daijun (and to recover from the purple mist in the latter; which is where I first died as an Assassin

Oh, and to avoid the missions that send you to take out a Naga Ritualist boss (the treasure recovery for the farmer in the Lagoon) and whichever one it is that sends you to rescue someone from the corsairs on the northwest tip of the Lagoon...

The main way for anyone to get a Survivor title is to just leech off other people. Stay back and let the rest of the team do the fighting. Makes me wonder what the point of the title is. It just says "Hey everyone, I'm a really good leech, but I'm not good for much else."

Its not that hard to get rank 1 in Factions,
Just make sure you get Max armour at K/center..
By doing the quests there and at top of Bazzar you will get survivor r1 easily.
Depending on the Island quests you do you and yes you can be level 17 easily
Ensure you take every secondary profession do all those leaving the one you wish till end this boosts you. also if your finding it hard leave areas with level 10 hench and walk back the way same for any mission area on Island take your hench and go back for quests.
To many people want to Rush...by rushing you Die its that simple.
If you have other campaigns then its easir still,

With this you are quite prepared for the first steps on the mainland.
