Ok I have a NVIDIA GeForce 8600 GT with the updated driver, DirectX 9c, and running Windows Server 2003, and I am getting the Unable to Initialize 3D output error.
Its my roommate's computer, so I can't change the OP, but I wanna say its cause of the Server 2003, but I was able to run GW on his laptop which has Server 2008 but has a more outdated video card than this computer.
Any Opinions/Solutions?

Wow, he likes the server OS a lot huh? Server 2008 on a laptop, I like that.
Anyways, try actually reinstalling the updated driver itself, if it dosent work, try a registry cleaner and then try to reinstall if it still dosent help, try rolling the driver back a version or so and see if it helps.
I'd also try to look at reinstalling DX9 or maybe seeing if there is any updates to/for DX9 at all.
