75% of the time I tab out of GW and back in, GW starts acting choppy ( low frame rate even though it says my FPS is normal ) I don't know what is causing it. An explanation would be, I move normally, but everyone around me (if I'm in town) or my heroes/henchmen move without moving their legs, like they are teleporting.
System specs :

I +1 to this problem, makes me want to rezone.

It tends to max out the processor when tabbing in and out. Like has been said, there's not much that can be done about this. Seems to happen on every system regardless of hardware too.

if find that if i go to settings and set it for lowest graphics then back up to highest graphics, its normally fixes it but it doesnt always, zoning also sometimes works.

Only happens to me when I alt-tab out. Minimizing seems to fix it for me.

As you know, even in full screen mode, GW has the little minimize, window, and close buttons at the upper right. I suggest that, instead of using ALT-Tab, which minimizes GW, you use the "window" button instead - I've never had the "choppy" problem when going from a window to full screen. Or use the minimize button, instead of ALT-Tab.

Use window mode and if you have a wide screen pull it until it fills your desktop. Works perfect.
