Health Bar Percentage Indicator texmod

The link on the wiki isn't working, can anyone who already has the mod upload it?

Since nobody else has replied yet, I thought I'd tell you how I do it without any mod. I simply place the health bar right above my 8-slot skill bar. This way I can see when it's below 50% when I see it goes to the left of my 4th skill on my skill bar. This gets more tricky when the target is affected by Deep Wound but, in that case, I consider the left of the 3rd skill slot to show when it's below 50%. This is what it looks like:
