Brawling Headbutt or Club of a Thousand Bears?
On my assassin I prefer to use CoaTB instead of BH especially with Moebius Strike. BH is listed on quite a few PvX builds, but it's a touch skill which limits it in some ways. I don't quite see the non-human thing as too big of a factor with CoaTB since I'd simply wouldn't use it in areas with humans, but it's damage depends on the number of foes in adjacent range.
I would think CoaTB would at least get some attention outside of an occasional warrior I see.
The main draw to them at least to me is their knock downs.

How large are the groups you're facing? Club of a 1000 Bears gets stronger with larger groups.
What adrenal skills do you have on your bar? Brawling Headbutt interacts badly with them.
How quickly do you accrue adrenaline? CoaTB is limited by a long recharge; Brawling Headbutt potentially charges quite quickly.
How heavily have you stacked damage buffs onto your attacks? Brawling Headbutt is a touch skill, but does a fixed amount of armour ignoring physical damage.

Presumably you can charge 7adr in less than 12 sec. So, BH wins.
Exceptions would be Moebius Strike and situations where BH would mess up your adr chain for more important things, like SY!

So that'd be on Dervishes and Warriors and Assassin's with Moebius Strike? Which is more than half of all melee options?

The only place I've run Brawling Headbutt was on a Dragon Slash bar because you have so much adrenaline and you can qknock things if need be. Otherwise I just can't put it on one of my bars without thinking I have something better I could be using.

I use brawling headbutt on my Locusts Fury assassin bar, couple that with critical agility and you have a knockdown every 4 seconds or so, it has worked well for me so far.
