How exactly does the ladder work?

Ive searched everywhere for the answer to this, including the search on these forums and turned up nothing. How, exactly does the guild ladder progression work? What PvP matches determine movement up or down the ladder? Is it specificaly guild hall battles? Battles in the tombs and beyond? Or are the arranged teams battles a factor as well?

Im asking because I see on the ladder that my guild has a couple losses, but I (the leader) dont have a guild hall, and have never been to the tombs to fight, only in random teams arenas and arranged teams arenas. Could one of my members be messing up our place on the ladder somehow? We are definately not ready for ladder competiton yet..

Only Guild vs Guild matches count, and you can fight these only from your Guild Hall. Are you sure you don't have one?

I dont see how i would, unles my guild went and got one behind my back and didnt tell me.

is a guild hall an actual tangable place you can go to?
If so how does that work? isnt there like 100s of guild halls then? where would they put all of them/how does it work
