Does anyone have or know of ...
1. A listing or description of how or what needs to be done in order to get from point A to point B.
I made it to Yaks Bend by pure luck.. now I am kinda stuck unsure of whats the next step ie.. how to get to next city / Vendor Area for upgrades.
Any help / hint is appreciated. If this is too much of a noob question, I appologize, I will figure it out eventually.

Do the quests given to you in Yak's. That will lead you to discover Borlis Pass. There are missions there that dump you into Frost Gate.

There are two ways to progress through the game -- going through Explorable Areas, and beating Missions. Missions are technically the game's story, so I'd reccommend you use those to progress on your first time through ^^;
There are often quests in outposts that will lead you to the next mission, so try playing a few of them.
On the other hand, if you just want to see where everything is so you can get there yourself, this topic might help.

Thanks to everyone that responded, and after reading my post, I am not sure I was clear on what I am looking for. I guess I am looking for a "Flowchart" type document.
There are a few maps out there, none as nice as these and local I might add. I understand the Mission idea, and thats what I am interested in, however the Exploration (is not a feasable option as a solo Monk even with Henchmen), I have tried it...
I guess the goal is - What are the Steps / Requirements to reveil and add a new location on the travel map?
I am looking for something such as the following:
If my goal was to get to Lion's Arch, I would have to complete the following Quests and/or Missions located in these cities / areas:
Example:
1. Start at "City/Area" do the "Mission/Quest ?" this will reveil / add "Area/City" to the travel map (which I can travel back and forth too)
2. From ? complete Mission/Quest this will reveil / add "?" area to the travel map
3 blah
4. blah
5. blah
6. City/Area -> Mission / Quest -> Lion's Arch
If I could remember how I got where I was at the moment I would post a real world example.... lol... I think in the progression of the game, I am in-line and currently at Frost Gate. /shrug
Obviously, there might be some areas that you just have to "explore" to get there, and thats fine. But most I have found, a "mission" is the best approach not only for the storyline, but for the exp as well.
