Theres also another rare armor called "bloodstained jerkin". I have found this from the frozen chest in dreadknoughts.

Actually I found the monsters that drop phantasmal armors. Its in Uw after you do the first "mission" in there it is at the statue u come across. There is a quest where u go to the town and on the way to the town there are things called Phantasmals. i got 3 armors to drop from them so i dont think that what your talking about is very rare.

Those chests in Dreadnought's sure did drop some unique items, pretty much anything could drop except Red Iris Flowers.
