Idiots aren't just confined to Crystal Desert. I find plenty of ascended lvl 20 idiots in Ice Cave of Sorrows and Iron Mine as well; many of these un-infused noobs insist on taking Mursaats head on. And bear in mind, these idiots are just gaming idiots, in real life they may be doctors/lawyers/rocket scientists with higher IQ than you or me.
Temple of Ages is not just for lvl 20 ppl, otherwise A-net would've put in lvl 20 henchmen there; as for now, henchmen there are only lvl 15. But I do agree UW/Fissure are too hard for anyone who is not infused (wonder why one needs infusion for going there, but yes you do need infusion) and who don't have proper group tactic training.

I have done 2/3 of the acension missions, with my two brothers and henchman (we are warrior/nec, mesmer/ranger, ranger/mesmer, so we take the monk hench etc). We failed the second one twice at different points, both times we beat that point when we came back.
I am surprised to hear this is tough, but then again, I dont consider myself or my bro's as idiots. It is still hard to see how people get stuck here unless they have a total gaming disability.

once you figure out the right strategies, they're not that hard (of course, you do have to spend some effort to find the right strategy)
too bad not henchies are allowed in the underworld!

Through all my quests on my 2nd ascended character I told people what to do and they still wouldnt listen. Told them where to hold, where to move on the radar and some idiot would allways scribble or start ringing a mob which wasn't to be touched.

I formed a PUG with two friends, a 20 El/Mo, and a 19 Wa/N and some others we found in the zone. One warrior joined, and somewhat rudely stated that our level 19 friend had better be booted or he was leaving. Rude as he was, we didn't comply, but another person in our group diplomatically convinced our friend switch to a level 20 ranger he also had. Somewhere along the line, we also picked up another level 19 though, so we headed out to UW with a full group with two monks. We were pretty much dead the second we got there. When we got back, the rude warrior said he would be happy to go again if the level 19 was kicked, as well as me, the mesmer, because "mesmers are useless until PvP anyway." Needless to say, he was kicked, but it really soured the experience for me.
So, yes, the level 19 probably has no business being in there just yet, one extra level isn't hard to attain, but try to be diplomatic about things. And booting people because they aren't a Warrior or a Monk isn't very cool either. I'm sure you're very polite, though. Its just some people who have a lot to learn and really kill the experience for everyone else :P

Conversely, both the Underworld and Fissure are quite manageable with parties of six. You need very skilled and coordinated players...but that's a good yardstick for finding pick-up groups there. If someone demands level 20s with full att points and so many elites then you know he probably hasn't gotten far (or at the very least he was riding on a guild group's coattails).
I guess I just expect players to be good. I expect them to have ascended and I'd hope they finished the game. If they're going to the underworld or fissure to collect ectoplasm and shards at level 18, they probably aren't the type of player I'll enjoy playing with. That's probably the necro with a hammer or the monk who uses a bow. Or Ice Smiter.
