Lately I'm having a problem where I will click on a monster to attack, the screen jumps to me standing right next to the monster and then jumps to me attacking the monster.

Happends to me mostly when I ALT+TAB/ALT+ENTER but goes away after a while or when I zone.

It is lag. Check your connection, firewall, and find out if your ISP are having any problems on that.
I've noticed on some PCs that it can also happen as graphics are being drawn in the distance, but that could also be connection related as I think some textures, models and AI pathing etc. need to be tracked by the server.
