What exactly is the death penalty in this game? Is it harsh?
Auh
No. You only get a death penalty if you die in a mission or PvP. If you return to town while you have a death penalty, it is removed. You are still sturdy after a death, but it goes downhill from there.

What is the death penalty? Weaker? Lose EXP? Lose items?
The only way to remove the death penalty is to go to town? And it gets worst the longer your in the mission? So you may have to leave near the end of the mission?

There's no permanent loss at all. You don't lose items, gold, XP, or anything else when you die. If you're alone or you're the last one in your party then you'll return to the nearest lobby or resurrection shrine (Also, in the Pre-Searing Tutorial, there's no DP gained when you die, either) where you can try again. So when you die in Guild Wars the only thing you lose is progress.

If you continue to be killed you will recieve more DP up to a total of -60%.
Then even if you die again it not go higher.
This penalty can be worked off by killing opponents. Usually it is reduced 1% at a time. To completely remove it you can go to a town or outpost.
You will not lose any items or money at any time.
So, is it harsh? I think it is not.
I still try to avoid getting it though.

On a side note, you get a 'morale boost' whenever you kill a boss. The boost is something along the lines of +2-3% towards your health and energy pool.
