It's a max damage req 9 +15% while enchanted Sundering Mursaat Hammer of Fortitude. It has 10/9 sundering haft and +28 grip of fortitude.
I don't know if there is more than one skin possible for hammers but this is the skin that is a bunch of stones seemingly floating in the air.
