I'm standing by brother Tosai right now, wondering what to do. What happens when you drink the chalice?

What are the pros and cons about drinking or not?

Drink and you'll get a couple of pip health degeneration that can only be removed by continuing with Tosai to Undercity and killing multiple waves of Afflicteds.

Not drinking makes surrounding Am Fah aggro or somesuch, haven't tried that yet
