is there a list of the different titles, and what you have to do to get them somewhere?

You are about to get beat with the search stick. There is a list, and how to gain the titles, but where is beyond me. Try the search and when it fails, which it seems to do a lot, go to guildwiki and look there.

in the IGN article it showed a screenie with a monk who had a title listed how many quests they had completed. In this case if was 1000/1000...has this titled been overlooked or possibly just removed?

I do not believe that, I have completed EVERY single quest with my warrior except Defend North Kryta and the one after that.

Quest title was probably a beta title that was simply ex-ed out before the release of the game.

Plenty of stuff gets added and removed from beta to real game.
