Well, seeing as the first reply wasn't very helpful, I'll answer...

It happens when someone calls a target. Either click that icon or hit the T key (by default) to select their target as your own. This helps focus fire.

It means that whoever's name it appears next to has "called a target". Meaning that they have pressed Ctrl and clicked on a certain foe to let the rest of the group know what they are attacking. It's useful so that everyone in a group attacks the same thing. The "T" is to let you know that if you press "T" on your keyboard, you will target that same foe.

Target and enemy by holding ctrl then attacking them, it will show that target. Anyone else can click that symbol to target same enemy. It makes it easy for the rest of the team to all attack the same guy.

Fifteen Over Fifty [Rare]

There are actually three variations of the target symbol (unfortunately I don't have pics atm):

1. Someone calls a target by Ctrl-clicking or Ctrl-Spacebar while targeting an enemy.

2. You're targeting someone else's target by clicking on the symbol, pressing T, or else targeting your own called target. The symbol changes to show that you've selected that target.

3. You're attacking a called target. That's when it shows a T over the symbol.
