A PvE character can do both, the PvP character option lets people start at level 20 so they can PvP competively right away but those chars never get to do PvE.

As far as your secondary, part of the fun of the game is that any combo can be effective but if you want to be a dedicated healer with protection buffs you will likely be using almost all your atribute points in Divine Favor, Healing Prayers, and Protection so whatver your secondary is it will probably usually have a minor role in your build so look over the abilities list paying attention to skills that don't require a high attribute to be useful.

that kinda helped... but what secondary works best for a healing monk?

You can get some benefit from a mesmer secondary even with few attribute points from skills like Chanelling, Power Drain, etc- these will give you extra energy to cast your healing/protection spells. Even at low attribute levels you'll gain a few energy from these skills.

Warrior secondaries can help out with just a few points in tactics as well, depending on your play style- if you get in a few hits with your wand occasionally between casts, you can use some nice skills like "Watch Yourself" to reduce the damage taken by your party. Some of the other shouts can be nice as well, and stances can help keep you alive. Watch yourself has a 5-second duration at 0 tactics IIRC, and if you're attacking constantly you'll gain your adrenaline for it before it wears out. Even if your only attacking rarely, it will build up, and you'll get a free +20 AL for the party occasionally, without any energy cost.

Elementalist secondaries I'd only really use for glyph of lesser energy if you're not investing many points into an attribute- some of the earth skills are nice, but they're generally only mediocre at best without some attribute levels. Still, getting off an expensive spell like Shield of Deflection or Healing seed for just an extra second on the cast, and at 10 less energy can be nice.

Rangers and Necros aren't really great without attribute expenditures- they do have some skills that can help out, but in general you need to have some attribute points spent in order to make those skills worthwhile. If you use ranger, you could dedicate a skill slot to Charm Animal and bring in a pet to help tank and deal some damage, but you'll have to watch out for his health, since if he dies it will lock out your skills for a while, preventing you from healing.
