I monked for a 4 man SF team today and we got really far. The other three players were a fire nuker ele and two wammo's.

My monk:

Mo/N
16 divine favor
10 prot
9 blood

Reversal of Fortune
Mend Ailment
blessed signet
balthazar's spirit
offering of blood
holy veil
life bond
divine boon

Life bond everyone, balth's and boon on yourself, holy veil everyone so that veil gets stripped instead of the bonds (also for hex removal and so that you get more energy from blessed signet). Make sure your warriors tank. Don't aggro anything yourself and if you do, kite it into the warriors.

You should be able to keep everyone fully healed using boon with RoF and mend. You should have plenty of energy flowing in via balth's spirit + life bonds, oob, and blessed sig.

I had no trouble keeping my warriors up even with sometimes three mobs aggro'd. If they'd had balanced stance we would have had no problems. When my ele got a good amount of aggro it was a bit tought to keep him up.

The fact that you had two W/Mo's shows your team build was inefficient; for farming teams, especially in SF, you only need 1 (gear trick anyone?). Toss in another nuker and you're set. Balanced Stance is pretty much useless in SF for a tank anyway.

For energy management, Essence Bond on the gear tank usually helps. Since the gear tank will be the one you're protecting, Dwayna's Kiss ain't a bad heal to toss in there too. Mend Ailment only fixes Bleeding...it's not that big of a deal, and it gets reapplied too fast anyway. Bath's Spirit...you shouldn't be hit, not in SF anyway.

Apply life bond to everyone else and balth's spirit to yourself. You get energy each time they're hit. Dwayna's kiss is a nice thought but I have no points in healing. With divine boon and 16 divine favor, I heal for ~120 for every monk spell I cast. That makes mend ailment and RoF very nice, spammable heals.

Hmmm, missed the part about stats setup.

The thing is though that there's pretty much no point in Life Bond/Balth's Spirit combo if the gear tank's the only one that gets hit. For a normal team that'd be fine, but in SF there are gears, which make tank aggro about a thousand times better. With 16 DF, spammable skills are better. There's no point in the mass enchants on everyone though.
