Hexes don't stack with themselves, regardless of the source. (Neither do enchantments or conditions, for that matter.) Casting Conjure Phantasm on someone who's already Phantasmed, whether it's from a second mesmer or your own Echo'ed Phantasm, will just reset the duration.
If you want to make someone degen faster, try stacking Conjure Phantasm with Phantom Pain.

True, enough stacked degens will totally nullify a Healing Breeze. But it's a lot more efficient to just carry enchantment removal...
