Quick Question on Hexed Dmg

For Warrior weapons and Bows, how does perfect dmg mod go for versus Hexed and while Hexed? I always get them confused.

It generally goes like this for damage mods:

If it's a positive condition (>50% health, while enchanted, vs hexed foes) then it's capped at 15%.

If it's a negative condition (<50% health, while hexed) then it's capped at 20%.
