1.) They aren't always holding one, but sometimes you can see a weapon or focus. If it looks unnatural, it's going to drop
This isn't to say when you go to FoW that all berserkers will drop Chaos Axes. This only applies to monsters that are different from their normal skins.
2.) If a creature is taking less damage then he normally should, say in a firestorm [no longer viable] or Zealot's fire [see firestorm] then he is holding a shield. Hopefully this info saves you time in areas like UW, if you're trying to figure out whether to kill the last aatax or not.

i can remember reading somewhere about this.
items that enemies hold are ALWAYS from their possible loot table. By no means does it HAVE to drop however. it is simply a possibility.
you can also tell when an enemy is using a vampiric weapon (the Shiverpeak Centaurs carry a lot of them). i've killed tonnes of them. never got vamp from em though

W.R.T. UW: In all the ~250 ecto I've farmed I've never [noticed] a smite crawler drop a different item than that he was physically holding in his mandible

I have noticed this but when ever I see the weapon actually there it pops off and is either purple or gold. I am not complaining, but I agree it's funny when you see a Grawl having a hammer and a shield at the same time... OMGZOR they HAXOR!
Oddly in the grawl situation sometimes its the hammer that drops O-o
