You can assign up to 12 points...then you can get a +1 helmet bringing it to 13, then you can get a +3 rune bringing it to 16, then I think you can get a +1 weapon and a +1 focus bringing it to 18, is this correct? any other possible way to increase?

Methinks you're talking about attribute points and not skill points.
12 from attributes
3 from a superior rune
1 from helm
possibly 2 from a skill
possibly +1 from a weapon OR offhand (if there's a +1 on the weapon, there isn't one on the offhand and vice versa)
So the total is +16 always, +19 occasionally for certain attributes.

You can't get +1 Weapons and +1 Offhands from the same attribute line.

For weapons, there's the +1 to Axe Mastery, Swordsmanship, and Hammer Mastery. For shields/offhands are the +1 to everything else that has a +1: Tactics, Strength, and practically all the caster attribute lines.

One big "rumor" I guess you'd call it is that the game doesn't see any attribute level past 18. So though you can hit 19 on some occasion, its no different stat wise than 18.
This might be a fact - or just that the difference in level plays a littler and littler role in the power of the skill the higher it goes, and its a coinadence that 18 to 19 just doesnt make it any more powerful.
