I wanted to edit this thread and add another item but you were too quick with the reply ;-)

Sundering Twin Hammer of Hammer Mastery
19-35 (req 12)
Dmg +14% while health is below 50%
Armour penetration +10% (chance: 6%)
Hammer Mastery +1 (12% chance while using skills)

Seems like a good catch but if the hammer market sucks that much... :S
