My first death EVER? I think I was pretty careful about deaths; the very first one I remember having was in Post-searing when I accidentally stumbled into a nest of devourers.
There might've been others, but that's the only one I remember.

First Character Death Ever: Aggroed way too many grawl while on the way to baradin's estate.
Deaths since introduction of the Survivor Title:
Ritualist: Killed in Visnah Square ~200kxp
Sin: Smacked upside the head by the L12 yeti outside shing jea on his first outing.
Ranger: Killed in Byway by jade brotherhood team while using a 3hench team pre-heroes. ~400kxp
Mesmer 1: Killed while capping Searing Flames ~750kxp
Mesmer 2: Deliberatly BiPed himself to death after achieving LS the old fashioned way.
Paragon: H/H's got into a knockdown/daze lock while killing Abadon ~300kxp.
Dervish: Wiped while H/Hing Troubled Keeper ~400kxp

From a Flame Geyser during The Necromancer's Novice as I remember not knowing exactly what I was doing and skipped the part when reading that said I had to use Animate Bone Horror to set off the traps.

My first legendary survivor died in Gate of Madness mission on the hands of Shirokens (lagged, I flagged the heroes and went for the cut scene but when I came back I was all alone ;()

My cantha ranger was well, sort of trying for some survivor rank.Unfortunately that was ended on that first co-op mission ;P or should say "BOOM!" - "geez". Paragon was bagged by those mesmers near kamadan, i think, we aggroed a little too much - yeah. Necro (2 years ago) was sliced & diced by the lvl 10 charr warriors , after that, that was happening nearly all the time.. eh.

My first death was in the Catacombs. I was doing the quest for Necromancer as my secondary profession and I didn't realize I was supposed to make a minion to set off the exploding things. I just walked in the direction of my quest arrow and the next thing I knew I was dead.
Once Factions came out, I found a White Tiger and let him kill me over and over while I went AFK overnight. When I woke up the next morning I had a level 20 Dire White Tiger to tame and leet deaths.

For my survivor, hydras in Ascalon. I was walking with a guy and he was fighting. I was standing out of battle. Next thing I know I'm lagging and then dead. ._.

On my necromancer, might have been the traps in the necromancer quest in pre-searing, or just a grawl or something... can't remember. Still haven't died on my survivor mesmer, but I'm not doing it seriously so... if I die, won't matter. (If I die close my next rank into it, might be different though x]).

post searing..feb 05 beta..was running with a buddy through pre-sear and took them post. (2 rangers).... post searing did not act/look at all like it does now. Walked out the door...was on my way to sardelac sanitarium (it looked more like a wizard's tower then).... we ran into the ranger grawl boss and friends... it wasn't pretty at all.

My very first death happened while I was out playing with a friend in Pre-Sear... The friend that introduced me to the game actually.
Anyhoo, he went out, and said "See the bull over there? Try to kill it..." This was about 10 minutes after I started playing, and I thought it would anihiliate my character for all time.
Lucky for me though, he was showing me how his res sig worked.

My first survivor's "first death" was *stupidly on my part* a death in the ascalon arena XD
My last "first death," which was on my current war" was by a scorched seed and devourer "mini-mob" while I was running myself to piken. Was during a lag >.<
My current survivor *para*has yet to die, and I plan to go as long as possible without dieing till I have the FoW armor and Tormented weps I want on him, then i'll find some wacky way to die, and record it. ^^

On my 1st character it was with Rurik when we 1st see Charr, during the Charr at the Gates quest, that was me as wammo n00bsauce running in to "help" lvl 20 Rurik with my lvl 3ness getting pwnd
