The most consistent fail seems to be when I wanna go cap an elite skill, especially in a place like mineral springs, and either: A. Forget to change secondary to the elite skill or B: Forget to put in a cap sig altogether.

Most epic would have been zoning into a town during a 400 foe Vq......on purpose....because I wanted to remove death penalty....Yeah...I hadn't had my coffee that morning..Soon as I zoned, I screamed and slammed my head on my keyboard. Had to convince the dogs after that it was ok to come by me.
my epic fail? let's see...
i was playing AB with my ranger once, and i got into a situation that i couldn't handle. i was down to about 10 hp in my shield set, and a burning hit me. unfortunately, my shield set has -5 energy, and i didn't have enough to use mending touch. so without thinking, i swapped back into my bow set to gain back 5 energy, and immediately died because of the loss of +30 hp from the shield.
oops.

Heard a bunch of stories along the lines of "was new got a black dye, didn't know value, sold to merch for 1g"
I don't really have a fail other than not pouncing on lucrative farms when they were still viable but then again this means that I actually worked for my stuff. :P
oh, and dying bags. (it was just green but it was still a waste)

My epic fail?
We reached the "bridge of doom" in Urgoz, and someone said, "that's the bridge." For some reason, it just didn't look right to me that day, so replied, "no, it's not; the bridge is longer than that." Then I stupidly ran across and got the team killed. Don't know what I was thinking.
I'm still teased for this sometimes...

The worst one I have is from a long time ago.
I racked up a few deaths before I realized it.
I was in an outpost, I was about to make a new build for my elementalist as I was getting bored of that one.
I dropped my attributes to 0, took out all my skills, got sidetracked and left without anything.
I resigned, put a build back together and left again. 20 minutes and 30 or so deaths later, I realized I'd forgotten to set my atributes. >__<¿

Nightfall was less than a month old (that's very important, as it is the key to the epicness of my fail), and as I'm working my way through Vabbi, I come upon a Sandstone Crag and a Shambling Mesa. They were the first of these critters that I've ever encountered. No biggie, they die like anything else. One of 'em dropped an Ele sword. "Ooh! What's this?" I think to myself. So I ID'd it and then equipped it. Took one look and thought "Who would want this ugly POS?" Merched it, only to find out the next day that I could have gotten 600-700K for the thing. /facepalm /D'oh /fail
Buying an Unded Greased Lightning for 2 Unded Vizu+500e.
Although this was years ago, when the price relation of zippy to vizu was alot different.. I will always regret not having those extra vizus now hehe.
Another fail was selling bunches of rare pets to buy z10 back when keys were 6-6.5k each.

About 4 years ago I deleted a character to make a monk capable of farming UW. I ran him through Prophices in 8 hours had capped several elites and had max armor runed up for doing 55 run in UW. Next day my guildies wanted to do some Tombs/HA went to reroll my Pvp character and deleted my monk instead. 

-Making a survivor char and trying to attempt to get it maxed, only to delete after i was about 200 more pts to max survivor. I didnt like the name 
-Accidentally trashing my obsidian gloves a few mins after i got them. I always liked obsidian armor for mes so when i got a bds i sold it and someone gave me a armor run. After i got the armor i went to my gh to do Slavers with guildies but wanted to dye my armor so i did. I have mostly all armor on my mes and dyed them all black because i like it
. I bought some 1k gloves a few days before to match a certain armor but i was rushing so i could do dungeon with friends so i didn't read properly and i trashed my gloves to only notice a few hours that they were my obbies

just this week.....bought new armor for my monk.....thought I had changed (had low level stuff on)...and salvaged it..only to find I salvage the new shoes I just crafted.....it was late and I should have already been in bed, serves me right.
like now---waaaaaay past time I should be playing (got my new shoes today and made sure 3 times before I salvaged the old ones)....wont do that again.

I was flag running right when Factions came out and was running up to the stand. I wanted to be ready to fight once I capped so I weapon swapped to my staff set to put up attunes. Well it just so happens this also drops whatever you are carrying. Unfortunately my guild didn't really learn from this, since we ended up repeating the error in the Factions playoffs against Rift which probably cost us the match-up.
Nothing too major, but I did merch one of my r8 zodiac hatchets... I also merched a 19/19 fire straw effigy shortly after factions was released. Both were the result of merching with lag. I still wish I had that offhand even if it wasn't perfect!
...and then there was that moment of stupidity when I just had to have a minipet after they were added to the game. 50k for a jungle troll definitely wasn't the best investment, but Bubba is still one of my favorite minipets... even if I've gotten several of his worthless friends and family for free since then!

VQ of Sunqau Vale - my BHA Ranger/Rt and 3 hero necros. Early on I lose a necro and I wait for hero to res. Nothing. I check their bars and only one has res - the one that died. Doh - ok - no problems - just soldier on, and when I wipe we will be back to full party after res. Turns out that we didn't wipe - just went slower. On the 2nd last group I lost another hero. Me and the last hero tackled the last 3 kappa - he died but I lived.
I am standing there with the VQ finished thinking wow I am great when I looked at my bar and it dawned on me that my skill 8 was "Flesh of my Flesh".
Talk about epic red face lols.
The other one was my warrior who loved his +5 energy sword so much (woot - 25 energy ftw) that he customised it to make 20% more damage. Sometimes I pull that req 8 Zodiac sword out of storage and stare at it and wonder at how dumb I was

.Hmm, the most epic fail was when I was running a HB monk in an ursan group against duncan (was before the ursan nerf ofc). It was my first time doing duncan, pple asked that one of the monks adds Protective Spirit. I say ok, I will. I add some point in the prot line, put in the requested skill and we go in. After we did all the dungeon and got to duncan, swapped spirits, one of ursans laying dead near duncan, the sv/swap necro asks to cast PS on her and tells she will ping when it's running out. I cast, she dies and asks me why i didn't put PS on her? I reply I did, we retry: same result. Then I realize the skill she is asking for is protective Spirit, while I brought and have been dutifully casting Spirit Bond... We had to do it the direct way after all.
I got some fails with the traveler. The first one when he was collecting herrings. I was too lazy to farm so I just bought the fins (or whatever it was) required to get herrings from a collector then ran to the collector: the char I was on wasn't a great runner and I was close to death when I got there so I fastly pressed the exchange button and got my ele 5 pairs of non max krytan gloves... I sometimes also merched the collector item traveler required either the day before he moved or just after buying them.
Another fail was at my first survivor. A guildie wanted to show some new weapons to us and to do so he started a scrim. I asked not to fight as I was on a half naked (was changing armor just then) survivor. Meteor shower hurts.
