What is your biggest failure

^same. Took me MONTHS to finish any of the campaigns. Meanwhile, my guildies were all in HM. Losers. I knew there was a reason I left that guild.

I remembered another failure.
My first "real" dungeon (in terms of being a completely self-contained task, not just an underground area) in EotN was the Bogroots one. I was doing hero/hench. We got through it, got the rewards for all three quests in the area, but the large number of ettins had taken a toll-- we had quite a bit of DP, several people had conditions we hadn't removed yet, and we were still healing up when I saw a red ping on the compass.
Since we had already completed the quests, I decided I didn't want to tangle with another boss and/or mob in our state, so I mapped us back to camp. Not having done a dungeon yet, I wasn't aware that the ping was not an enemy.
We had run away from the reward chest.
Luckily it was hero/hench so there was no one to be upset with me, we still got credit in the book for completion, and one dungeon reward obviously didn't financially break me-- but it was annoying to be that close to a reward after already putting in the effort, and just not have even picked it up.

It was my first Wintersday in GW and I needed to be able to get to Droknar's Forge to complete a Wintersday quest, but I was stuck somewhere in the desert and wouldn't have been able to get through all the missions in time and didn't have enough money to pay for a Drok's run, so I decided I'd try and fight my way down to it from Beacon's Perch with an equally newbie friend. He was playing a dervish so he had some heroes, but not many skills and no idea of what builds to give them. Neither of us had much idea of what classes we'd need to make an effective party even under the best of circumstances, and neither of us had been in any of the areas on the run before so we had no idea what to expect. It turned out to be six hours of hell that Sunday afternoon - Dreadnaught's Drift was a 60%DP kill-one-foe-then-party-wipe nightmare - and I couldn't believe that it was so hard. We were in sight of the Camp Rankor portal when my friend announced that he'd just realised he'd given his necro hero a full bar of ranger skills but not given him a bow. /facepalm

Long ago when I was still a nuhblet and apparently couldn't read. I had t make 4 seperate trips through the FoW to get my dervish a new pair of pants. :/
The first trip I misread the materials list on the Wiki and brought Leather Squares, not Elonian Leather.
Second I fell a bit short in the cash department due to paying for the trip.
Decided to sell some stuff and ended up selling my hides by mistake.
Third trip I got cocky and tried to finish all the quests before going to get my pants and ended up dead on the last one. :/
The fourth I wasn't messing around, I got the forgemaster and my pants. Then promptly mapped out. My roomate punched me for leaving him in the middle of an epic battle. xD

One I just remembered, I was slogging through the Gate of Madness mission and was really struggling and couldn't figure out why. I didn't realize that I had Koss as my hero with a pretty lousy axe build (Chahbek village had been a recent Z-quest where you HAD to take him).
I had a similar issue where I was struggling in Destruction's Depths forgetting that I had set my necro's up with lousy Blood Magic life steal skills (to take down the worm in the Heart of the Shiverpeak's dungeon) and forget to change their skills back.
Always check your heros skills and weapons...

My most recent epic fail (aside from my previous post) came last night during a run to Nicholas the Traveler on my 2nd acct. Went to Camp Rankor, filled out a team of Heroes and Henchies for the run. Got to Nick, realized I didn't have the Griffon Wings in my inventory. They weren't even in my storage, they were on my Spirit Spammer. Changed toons, put the wings in storage. Went back to my ele, grabbed the Wings and left Rankor. Got into the first fight with the Stone Summit (4 Giant Herders and a Dolyak Master). Party wiped 3x before I realized I FORGOT TO BRING HENCHMEN!

Aside from numerous merching of good drops, and failing to max any titles since starting, oh, years ago? I have too many characters who I flit between, and I'm daft enough to try and get four survivors simultaneously. Read: Many Restarts.
One survivor attempt sticks in my mind. Pre Searing Elementalist. Level 8, haven't done any of the quests save the first one to get a couple of skills. Man, farmiong like that is dull, particularly as there seemed only to be a bunch of noobs you couldn't pay me to char hunt with. I'm sitting around outside Ascalon near Gwen, just about to go kill the drake, when I made the cardinal error. I thought 'Oh, there's nothing here', and didn't map back when I got called away. I came back and realised that I'd been killed by a scale broodcaller. /Facepalm
The one character I have a survovor title on has a similar tragedy. First time I've played a Mesmer. I've done the title 'properly', not by farming, and I'm doing the Consulate Docks mission. My character flips over to level 20 and I get my 'Survivor'. JHooray, exultation! Then I run into a mob trying to get the bonus objectives, and die. So, while I did at least get the title, I still count it as a fail, because it was so senseless. I think I'd only done the mission once before, and a wrong turn meant I certainly hadn't faced all the mobs. What a moron.
