Did a vanquish of Sacnoth Valley with a full party of people during a double EotN rep weekend. We did everything, but saved the Charr stronghold for last.
We were running around trying to figure out how to get into the stronghold and I realized that I hadn't done the required "Assault on the Stronghold" mission, so we couldn't get in. I was beyond embarrassed.

Lol that makes me remind of when i was playing with some friends in a random mission in PvE, and i was learning how to weap-swap on my monk, using the F1-F4 buttons, but at the same time i used to press Alt to see allies on the screen..guess what happened when i swapped back to my defensive set (F4)
Now i've learned and i've put the Alt function on the X button

I had been doing kilroys brawling on my warrior, before i decided to do Hells Precipice with a pug, just so i could finish the campaign. This was the first time I bothered to do this mission on a warrior, but I had done the mission plenty of times before.
I was the pug leader, and only physical on the team. Some of the other members suggested we bring Winter, but I told them it wasnt really necessary; the titans armor vs physical isn't that big of a deal. We startet to play, and soon realized that each fight took a really long time. We never really manage to kill the hands and fists that spawn last...
That's when I notice I keep hitting for 1's and 0's. Maybe I was wrong, maybe we do need winter? I kept that thought to myself.
After pounding on that same poor fist of the titan for about 5 minutes, I realize my awesome axe build has zero points in axe mastery. I had put it all in dagger mastery, so I would do a bit more damage brawling. Very embarrassed, I figured it was best not to say anything. After all, the group still had patience after using almost 30 minutes getting to the first imps.
Off course we wiped by the very sight of one rodgorts, and the group disbanded.

My most embarrassing is when i went through FoW with all the wrong attributes and the wrong mask on my ranger. I had nearly all my points in BM and a bow use bar...and not enough in marks to meet my req. I was basically leeching, luckily no one noticed because i was still auto attacking and using my useless bow attacks...If i had a pet on my bar it would have been as bad.
Another time I merched my Urgoz Longbow. I had bought it when they were worth over 80k, it merched for much less...

Called a prot monk in HA a noob for using Shield of Absorbtion. Told him -5 damage on a hit is stupid. I didn't realize that it went like -5, -10, -15 etc. We argued for over 20 minutes about it.

Lol, done that too. I always thought Shielding Hands was better.

Was getting a mod off my oppresor's healing staff didn't know what I was thinking used a expert salvage kit and the staff broke........ yeah.

dang, i didnt know that either....but then shielding hands has 1/4 cast

I knew nothing of Attribute Point Quests, so for nearly two years, most of my characters were running on 170 attribute points, rather than 200, in fact my Prophesies Monk became Guardian of Cantha a while before she did "The Hero's Challenge" quest to get her last 15 (even though I had know about the point quests for a while by then).
I had been a guild leader for some time and thought that I knew most things about the game. I often helped new guildies to get max armor, saying something to the effect that I would be happy to give them the common materials, but they would have to buy the rare ones from a rare material trader as they were so hard to get.
...Then I saw someone in another guild helping a newbee on Alliance Chat telling them that using an expert or superior salvage kit gave a chance for getting rare salvage... I never new that, thought you could only get rare stuff from drops in HM (I had always used basic ones when not extracting mods, runes or insignia - to save gold, so I thought!).
I am sure there are probably other things that I am still blissfully unaware of... hehe.
