I'll be totally honest, I never made this build. I created it based on the Wretchman Drake's inital build, his post and his last post.

I've used each skill before, just not as the build I just mentioned.

Anyway, as far as the attributes are concerned:

Death: 9 (8 + 1)
Blood: 10 (9 + 1)
Soul Reaping: 10 (9 + 1)
Curse: 12 (10 + 1 + scar)
Domination: 3

I think this is how it would end up. Again, I just thought about the build off the top of my head.
