Maybe you need to like use all the items at once somehow, like equipt the bow and put the horn and jug in the pack and then drink the jug.

I have the survivor tracking one on my new assasin, it says you have to "reach 140,600xp without dying" it came up when i was @ around...

I did it with Spiteful Spirit, Enfeeble and Insidious Parasite

You kill the rockshots and stuff before the first boss and make a little army right at the beginning, the bigger you're army gets the faster...
