Prophecies, Factions and Nightfall = One Epic (a series of three campaigns telling one story)
All classes (Monk, Warrior, etc.) within P,F and N are of the same Epic.
Classes in one Epic can not have as a secondary class classes of another Epic.
So, let's say Chapter 4 comes around. It's the first campaign of a new Epic. Chapter 4 introduces new classes for a new Epic. By way of example we'll call them the Arsonist and the Yo-Yo Champion. Arsonists can have Yo-Yo Champion as a secondary. Yo-Yo Champion can have Arsonist as a secondary. But you will never see a Monk/Yo-Yo Champion (or a Yo-Yo Champion/Monk). If Chapter 5 is made, and it introduces new professions (again, by way of example, Newtonian Physicist and Girl Scout) you could make a Girl Scout/Yo-Yo Champion or a Newtonian Physicist/Arsonist, but not an Arsonist/Ranger or Girl Scout/Mesmer.
This would keep skill combinations from multiple classes from becoming degenerate due to sheer volume ... it's much easier to balance skills when the number of class combinations is limited.
Of course, with the power to fix the game via update at any time, A-net could do whatever it wants w/o doing any permanent damage. So new classes/skills aren't the greatest threat to game stability.
