With my Legendary-Survivor-to-be I was using auto-run (R) towards a mob of Dredge (with a warrior boss), and reached for the ctrl key and hit the Window button instead. My computer is slow, so by the time I got GW back up my survivor was dead.
Worst part was hearing him take damage through my speakers and not being able to do anything!

It was kinda funny yet heartbreaking, I had cleared a zone, so there was no enemies anywhere near me, and yet when I got back (went to take a phone call and do some other things) I was at a res shrine. Typing /deaths really sucks at 1.1mil xp.

AFK with a vamp weapon at 1.1mil exp was brutal, I sympathize. 
I have to admit that my biggest mistake was buying FoW for my monk back when ectos were 13k/e, then making a new monk for that legendary survivor title. Upon deleting my old monk, I figured I'd try to get what ecto I could back...
So for me, the pinnacle of crap was: Salvaging full monk FoW to receive...tanned hide.

Worst pwnage for me?
1. Lag spiked while applying dye and dyed my backpack black on 3 seperate occasions.
2. Went 55 in RA. My team was all assassins, they died quick. Other team was warriors and one unseen mes. -5 -5 -5 oh wait -300. dead.
3. GvG when I was younger, played with catapult and cost the guild the match by one point.
4. Lag spiked while monking and cast healing breeze on the MM's minions four times.
5. ^^Did the same with BiP only on a Warrior in the middle of Urgoz's. Died horribly.
6. Brought Cap Signet instead of res signet to TA match.
7. Sold a req8 Chaos Axe for 3k because I thought it looked stupid. (aaaggh)
8. Got scammed when I was a newbie by a Sin using that teleportation method. Accidently dropped my pants, Sin took pants and left.
9. AFk'd in Fissure while team leader went for a piss break. I accidently hit 'R' and ran into 30 kabillion enemies and killed everyone.
10.Last Wintersday I sold my req7 Perfect Crystalline for 100k and 1000 frozen ectos. I just saw ectos and hit the 'accept' button. I almost cried.

Not selling my 15>50 r9 Crystalline back when I was being offered thousands of ecto, but it's still just fine sitting on the warrior I never use.

I bought a few important runes for a character I don't use often, clicked to apply, and... d'oh. She already had them. >_< I don't even remember giving her any sort of valuable runes, either. It didn't cost me that much, maybe 1.5 plat total, but I was tired and still managed to apply one before realizing I had redundant inventory.

Going out to cap FoC on my ele for skill hunter getting there killing the boss, relizing I forgot cap sig quit go back to town. Head out again with cap sig only to repeat the same thing get to the boss and relizing I was the wrong secondary......needles to say I still dont have that skill.

Using Ignite arrows while using a sword, also using barrage with preperations and wondering where they were going.

1) Mesmer capping Aura of the Lich in Hell's Precipice. Got all the way to the boss, which was at the part with the Armageddon Lords (nearby) and it dawned on me that I was the wrong "green" color (Necromancer vs. Ranger) for secondary.

Wanted to cap Barrage, get a PUG, get about 1/2 way thru and 2 people leave and don't return. Get wiped.
Try #2 Another PUG. Get all the way to Markis, and DC (I'm on dial-up and it randomly dc's, not very often, but too much for me)... can't reconnect in time to get back to my party.
Try #3 Yet another PUG. Get to Markis, kill him, Jade spawn, kill Jade, mission over. No cap.
Try #4 One guy from last PUG. Said he'll help me. Get to Markis, Kill him, while the Jade beat on the nice helper guy, I finally cap my damn skill.
Wasted hours on something I shoulda been able to get in 10 damn minutes!!

worst self pwnage? running ahead, screaming "LEEEEEEROY JENNNKINS!!" against a spirit spammed enemy base right of the bat in RA. only did that once though, and my team was noobs anyways.
