Rumor (from Korean forums)
Gwen is dead. Sarah, her mother, is at the start of UW. Rumor has it that after clearing all monuments in UW, Gwen is accessable through the Citadel door near the smites. It follows that returning to Gwen four items will do "something."
Items of interest:
Pre-Searing:
Tapestry Shred
Post-Searing:
Gwen's Broken Flute (located near Sardelac Sanitarium)
Tattered Cape (located in Stingray Strand, south of Fishermans Haven near Mingas the Meek)
Dead Flower (unknown, rumored to also be in Stingray Strand)
I cannot find the "Dead Flower" but it has been mentioned briefly before.
It is my guess that you ascertain these four items, talk to Sarah, and then the quest begins.
Any of you have information regarding this?

I'm guessing this has a >1% chance of having anything to do with your 'quest' but in pre searing, there is a collector..or maybe merchant, sorry, who has a 'little girls cape'. No ideas what its used for as I didn't want to spend a bunch of gold on it.

If you give the Cape and Flute to Gwen in Pre-Searing she will follow you around and heel you 20 or so HP every time she plays the Flute. She doesnt incur the wrath of the monsters you are beating up so she can be left to her own devices, dancing and skipping about while you get down to business.

Pre Searing you can meet Gwens mum Sarah near Devona. She tells you about gwen always running off. You find the broken flute in the river near acalon, and when you give it to her she complains its bust. From then on you buy items for her, the new flute, give her red iris flowers, and buy her the cape. Its my understanding once you do this, she gives you a tapestry.
Once you sear if you revist the places just outside the sardalac sanitarium, you find another broken flute, The tattered cape i have also heard people talking about. The dead flower i have no idea about but it ties in with the iris flowers from pre searing.
I heard on another thread that the "Gwen quest" was actually broken/unfinished. Although i have recently heard people saying they found her in the UW.
Someone was saying she was rapped and kiddnapped by the Charr. Others saying she got lost in the confusion of the "Searing" and became a warrior (not likely).
If anyone knows anymore, I'd love to know.
Not sure if i want to know spoilers but atleast if its fixed/active

maybe she will be implemented in the story later seeing as she leaves more questions than answers. most things unfinished in games will be used later. i saved up for the cape in pre and bought it. i gave it to her but she says thanx like usual.

At this stage I'm going with the theory that the Gwen quest is chapter spanning, and we will see more of the quest next chapter.

All these items are here to unlock a secret profession known as "The Gwens".
I'll tell you how, listen carefully.
First you have to collect 5 flowers pre searing but don't give them to her, buy a flute as well, again don't give her anything.
After you destroy ascalon you grab the tattered girls cape and the broken flute, you run to the river to the south east and stand in the water for exactly 95 seconds. After the 95 seconds are up you quit the game (exactly 95 seconds). Next find rurik, talk to him 13 times, on the 13th time leave the window up and go open your fridge door 13 times (closing it each time of course).
Once this is done close the window and go back to the river. Stand in it and equip the cape as a weapon and the BROKEN flute as your shield. Equip each of the 5 flowers as your armor.
Wait another 95 seconds and quit your game again.
When you get on your char again it will have transformed into a Gwen.
Gwen is a very imba profession with such skill as "Randomly stand around not helping", "Heal 20 whenever I feel like it", and "Spin in a circle while the Me/W gets owned in front of her very eyes".
If you guys attempt this and mess up your cd will blow up and it will delete it's self from your computer, FBI agents will walk into your house and inject something into your neck making you forget your CD key if for some insane reason you had memorized it.
Everything you touch will turn to ash and jesus will spit in your face.

Ok, I was hoping to get more serious replies, but you do what you can.
At any rate, I have all the items, sans Dead Flower. I'll go look again at Watchtower Coast, as I think I found one there.
I'll take what I do have to UW pretty soon and see if its enough to trigger the events necessary.

Um If I remember right Mingas the meek is the NPC inbetween that place and fishermans haven. He does nothing, says nothing, doesn't drop the cape, and has nothing to do with any of this. And could we get a screen shot of the dead flower? I've never heard of it mentioned before, and I'm pretty interested in the myster around gwen too. If you don't know, just to save you time, to take a screen shot it's the print screen button.

Don't you just collect the tapestry, tattered cape, and broken flute then drop them on the ground in front of one of those god statues and bow...? I'm pretty sure I did that at some altar thing and gwen came up instead of a spirit and gave me a dead flower. If I recall it's just used once like a scroll and makes enemies easier to kill for like 10 min or what not, which is usefull in the underworld on the level with her mom, Sarah.

That sounds very very unlikely. Witch is why I ask for screen shots. No one's ever mentioned that before, and probly won't believe it till they see proof.

I well know how to take a SS. 
Mingas drops the Tattered Cape, its ALWAYS around him when you hear the familiar "drop item" sound.
Like I said, if I find the flower, I'll let you know. I have every other item.

Hey guys um.. I THINK GWEN IS THE SEER!
Maybe in Pre-searing, she transformed herself into a little girl in search for the "Chosen." Then she followed all the way to Post-searing and then eventually transformed herself back to Seer form. This is a bit far-fetched, but personally as soon as I saw the seer, I was like "OMFG GWEN!"

Here's my note on the whole pre-searing Gwen thing...
MEETING GWEN:
- As a RANGER, my first quest was to kill skale I think. Then I met up with Gwen who needed to get her flute, at which point she starts following me around and I can give her gifts because I'm such a pedophile. I gave her a ton of flowers, a flute, and a cape. I never received a tapestry shred.
- As a MONK, my first quest was to rescue Gwen from the skale. I went through a TON as my monk (I became obsessed with seeing how far I could take my monk on 0 deaths but kept playing around in the Northlands and other dangerous areas post-searing.. I have become quite the expert at pre-searing because of this) but NEVER was given the opportunity (or even found) to retrieve the broken flute. Odd things also occured with Gwen. There were times where she would already have a cape and it plain boggled my mind. Out of a number of times going through, only once did I manage to receive the tapestry shred.
