The name of the boss with Skull Crack is Thorgrim Beastslasher in the Iron Horse Mines- he wont use Skull Crack unless you're casting spells (obviously). He's also level 24 at takes forever to bring down. He uses Healing Signet, so make sure to have an interrupt.

ive found shield of judgment!! ive noticed it being used by many named white mantle in a few missions, but the only one i could ever use my signet on was the one from the sanctum cay mission, right after you find the vizzier. woot!

Shield of Judgement, Ineptitude, and Well of Power can all be captured from Tengu bosses in the Mineral Springs area. I didn't get the monster names, however.

Noticed that the dark flame dryders on the fire islands use Soul Leech, but couldn't find a boss related to this mob, maybe one in one of the missions?

I can confirm energy drain...it takes a while for him to cast it but if hes that purple/pink mesmer color he has it I think I was draining his mana and he used it on me

I'm starting to think Incendiery arrows is in Abaddon's Mouth, not Ring of Fire. There's a large portion of the island to the northa that I didn't explore.

I found a ranger skill call Spirit of renew. when I go look up it on the skills listing on the guildwar guru I didn't see it there.
it was used by a boss mob call Tree of renewal in the end of d'alessio seaboard mission, a spirit that reduce all skill cost by 15, it was elite as well with a gold border. not sure if anyone saw it.

there is a mininum skill casting associated with the skill, but I couldn't quite catch it because the info was too long and it was cut off... plus it was casting it when my henchmen killed it.

Crippling Anguish can be capped from the Yxthoshth (A Storm Kin Boss) in Salt Flats. However, it only appears for the Ranger's Path quest, and is surrounded by like 30 Storm Kin.
