I have a crude map of the fissure of woe created a while ago. (I downloaded it over 2 months ago.) Don't remember where I got it from....
Axe/hammer/sword/bow/staff of Defense is definitely 4-5.
Curiosity gets to me sometimes... I've salvaged iron from steel just to see if it would work, and I once also salvaged 4 bones from a scroll...
