The map shows every locations that you have been to. In order to have all the five cities on the map, you have to first visit them.
As soon as you are level 3, speak to Sir Tydus. Also, for the future, try to not skip the dialogues.
To leave old Ascalon you need to be at least level 3. When I left Ascalon I was level 7. To say the least, my opponents were surprised .
