Sword brings a lot of consistent damage through bleeding, but can't cause deep wound without the target bleeding. Axe and hammer can do that...

I have a 15^50 req 11 hand axe (white scythe skin) that has + 30 to health.

A mistake i noticed was that one certain build had the Lightning Surge elite as well as the Spiteful Spirit elite. Sounds like hax :P
