South of Lion's Arch is a big boat icon, use that to map travel back to Kamadem. Boat icon is just east of Kam if you want to go back to LA.
Reach lvl20 and complete the all the missions(not quests) for that campaign. After that, the toggle switch for Hard Mode appears in your party window.

I doubt the missions are available to pre-sear chars. Or even to chars below lvl 20, much like how Hard Mode can only be accessed by lvl 20 chars....
