If you use mending and holy veil take along CoP (contemplation of purity). Then you will remove 2 conditions and 2 hexes.

1) Take out heal area and put in penetrating blow. 2) Switch dismember with Eviscerate. 3) Take Watch Yourself to fill in that extra spot.
