It's not supposed to stack. The Runes are only used to lower your health. And you are probably missing the Attribute Point quests in Factions...

Pretty much depends on the item's and upgrade's values. I can only shake my head when I see "WTS 20/20 15^50 +30HP Dead Sword"....
