i can give you the most common combination. Warrior/Monk theres a billion of those around

it doesn't trigger it if it is not physical dmg (melee weapons and bows without elemental mods) chaos axes and the like doesn't trigger it...

they are imune to poison and disease afaik. they can bleed and become crippled. deep wound. and dazd (but hardly ever used)
