All roles are temporarily exchanged: Characters become either monsters or NPCs and monsters and NPCs become characters.
So you either sit around all day in a town feeling like:
Sisyphos, because you give away a quest to three dozens of characters to get the job done, reward all for doing it and finding out that you have to give this quest away to everyone else still (quest-npc) or
the ultimate altruist, because although you're a merchant - which is why everybody mostly ignores you while they interact with you - you buy loads of crap and end up with a HUGE deficit - your only good thing is that you have an unlimited supply of utilities like idents and salvage kits (merchant npc)
Or, you sit around in the explorable areas only able to move around by a few inches just to be slain over and over again (and maybe multiple times parallel, for multiple instances) by characters coming along. (monster)
This change lasts until you have completed the following goal:
Quest NPC: Have the job done once and for all.
Merchant NPC: Resell at least three quarters of what you buy from players.
Monster: Kill all characters in the game.
Of course, this has to be accomplished while standing around (or moving in a small area) in more or less the same spot all day.

The worst update was nerfing Shadow Prison and the A/E and E/A Solo green farming build. I hate how monsters such as Darkroot and Sunreach runaway when you use silver armor. That was screwed up. Plus Shadow Prison didn't need to be nerfed to the cost from 5 energy to 10 energy. That was stupid and pointless. If the idea was to nerf BOA Shadow Prison Sins, that was pointless because the build is overrated itself. However, Shadow Prison does come nicely with some pve builds.

SONIC mode.
You only have 1 life. If your life reaches 0, your inventory drops on the ground and then quickly disappears. If life reaches 0 while you don't have inventory, you're dead and have to start anew.
IRONMAN or PROFESSIONAL CHARACTERS mode.
If you die, your character (or account?) is deleted.
ALL YOUR BASE ARE BELONG TO THE UNSEEN TORMENT CREATURES mode.
Every monster is Level 28, can do "Call to the Torment", can do "Spectral Agony" and it is impossible to infuse ANY armor. Present infusions are undone.
MONSTERS OF THE WORLD, UNITE! mode
All monsters are considered one group - aggro one and you aggro every single creature in your map.
MOTW, U! - hard mode
All monsters from all areas in all campaigns are considered one group. Trigger that devourer right outside Ascalon, and Abaddon, Glint, Kuunavang, the Lich, Shiro Tagachi as well as every single high-level creature will come to get you. They will automatically spawn in your aggro radius to keep you busy while the creatures from the current area have to walk towards you.

I'm one of those sorts with a mirror personality. Whatever gets directed at me gets reflected away from me. When I feel that things are against me, I tend to be against things in general.... I don't mean anything too personal by it.
But that doesn't mean I get in the slightest why anyone would want to make things that difficult for themselves. I can only assume it is because they don't take the games seriously at all.
I find if games are too challenging it seriously stresses me out... and can make me physically sick. This is quite ironic, considering that gaming is my best means of relieving stress in the first place.
I'm an obsessive-compulsive gamer... and unfortunately I need to try and limit myself to things well within my capabilities to avoid stress. It just makes matters unpleasant for me when everyone else is demanding that things be placed way outside my reach.

THAT is what I have an issue with.
It is something that hasn't been too much of a problem in Guild Wars so far.... Or at least not in that collector weapons can often be made to roughly the same standard as elite weapons gained from special sources. Admittedly quite a few of the special weapons do actually manage to be better than any collector weapon could be.... but it isn't by much.
But effectively... I know there is a limit to my capability.
And since I will never be able to surpass that limitation (it isn't just a matter of effort), I know that some aspects of the game that actually make a practical difference are inherantly denied to me.
I mean... if Hard Mode were only a means of unlocking certain things FASTER or in greater quantity.... then I wouldn't have a problem. I would have an alternative means open to me... provided it was within reasonable limits. I have no issues with games that have multiple difficulty levels that just quicken the process.... or give a challenge for its own sake. I just don't bother with those aspects....
But when there are certain practical items unavailable to me because I am not physically able to deal with the means of aquisition.... That is when the game has gone too far. That is the sort of thing that causes me the stress in real life that I go to the games to relieve....
Plus there is the horrible issue that certain aquisitions in Guild Wars do make a practical difference not based on game mechanics. I have realised that on the odd occasion that I need PuG help for some reason.... that I will get that help more effectively if I have a shiny title and a set of 15k Armour....
The worse side of this is the elitism... and it is truly rampant... I'm no stranger to it myself in other media.... But it is wholly unjustified for some idiot who can't even type to think himself better than me just because he has a shinier sword.
That is the sort of thing this Hard Mode is going to promote.
People who actually WANT a challenge shouldn't be rewarded for doing what they would do anyway...
