For PVE it's usually enough with Cruel spear/slayer's spear or merciless

Prayer of the Forgotten Cruel Spear +Fortitude since I use deep wound skills a lot.

Seems like no patient ever get cured and did not become afflicted.

Why shambling horror? With jagged bones vampire horror or bone fiend is better

And I think Animate Bone Minions is better than Bone Horror since it can trigger BoC and EG twice.
