You can't 600 smite it. Fingers of Chaos = For 5 seconds, your attacks: cannot miss rangers; remove enchantments from monks; cause conditions...

in HM over 3 enemies = scatter. NM is better for SS/55. Look at the link that MisterB put, that will answer all your questions.

greens are just a random drop. If you see the Boss holding the weapon he does have a higher chance of dropping it though.
